How much could care for this joint cost? Office care and surgery bring very different bills. Your health plan may pay some charges and leave others to you. First, get the exact name of the care being offered. Then ask the clinic and your health plan for written costs before you agree. ## Which charges belong in the estimate? Surgery may bring separate bills for the surgeon, anesthesia, the facility, a brace, later visits, and rehab. MACI means matrix-induced autologous chondrocyte implantation. One operation takes cartilage cells from your knee. A lab grows them, and another operation places them into the damaged spot. A graft is a plug of cartilage and bone moved from your joint or from a donor. Ask whether each stage and graft charge is included. A written total isn't complete if one service is missing. ## What do I ask the health plan? Call after you know the exact care being planned. Ask if the plan must approve it first. Also ask whether the plan has agreed rates with the clinician and facility; that is what in network means. Coinsurance is the share you pay after meeting your deductible. A service the plan excludes is fully yours, which is different. Record the date, the representative, and the reference number. Keep those notes with the clinic's written estimate. ## What do I need before saying yes? The clinician needs to examine the sore joint and compare the findings with your scan. Ask whether the care aims to ease soreness, improve movement, reduce catching, or repair a damaged spot. Ask about recovery time and how you will get around at home. List your medicines, the tasks you can't do, and earlier care that did not help. The NIHR health technology assessment of autologous chondrocyte implantation reviewed the trials and built a new economic model. Survival analysis suggested better long-term results with ACI than microfracture, and in most analyses the incremental cost-effectiveness ratios for ACI versus microfracture fell within a range usually considered acceptable. The assessment flagged the absence of randomised data beyond five years, and that the long-term data come from forms of ACI now superseded. FDA biologics licence BL 125603 (Vericel) describes MACI verbatim as "an autologous cellularized scaffold product indicated for the repair of symptomatic, single or multiple full-thickness cartilage defects of the knee with or without bone involvement in adults." The licensed indication is a two-stage surgical implantation for full-thickness DEFECTS of the knee in adults. It is not an injection, it is not licensed for osteoarthritis, and it is not licensed for any joint other than the knee. FDA's consumer alert states verbatim of stem cell products, exosome products, adipose-derived stromal vascular fraction, umbilical cord blood, Wharton's Jelly and amniotic fluid products: "None of these products have been approved for the treatment of any orthopedic condition, such as osteoarthritis, tendonitis, disc disease, tennis elbow, back pain, hip pain, knee pain, neck pain, or shoulder pain." The only stem cell products carrying FDA licensure are blood-forming cells derived from umbilical cord blood, licensed only for disorders of blood production, and there are currently no licensed exosome products. In orders issued 26 December 2024 and announced 8 January 2025, a federal court permanently banned the co-founders of the Stem Cell Institute of America and related companies from marketing regenerative medicine treatments, and ordered $5,155,146 in civil penalties and consumer refunds. On summary judgment in March 2024 the court found they had published false and misleading advertisements about the efficacy and approval of stem cell injection treatments for conditions including osteoarthritis and joint pain; one clinic charged up to $5,000 per injection to a group of consumers who were almost exclusively elderly and disabled. PALLIATIVE chondroplasty outnumbered marrow-stimulation repair by more than 2:1 and outnumbered restoration procedures (ACI, osteochondral autograft and allograft) by roughly 50:1, regardless of age, sex or region.
