FDA biologics licence BL 125603 (Vericel) describes MACI verbatim as "an autologous cellularized scaffold product indicated for the repair of symptomatic, single or multiple full-thickness cartilage defects of the knee with or without bone involvement in adults." The licensed indication is a two-stage surgical implantation for full-thickness DEFECTS of the knee in adults. It is not an injection, it is not licensed for osteoarthritis, and it is not licensed for any joint other than the knee.

The only stem cell products carrying FDA licensure are blood-forming cells derived from umbilical cord blood, licensed only for disorders of blood production, and there are currently no licensed exosome products.

4. Forty-five people who had had a partial medial meniscectomy 3-5 years earlier were randomised to four months of supervised exercise three times weekly or to no intervention. The exercise group improved cartilage glycosaminoglycan content measured by dGEMRIC relative to controls (+15 ms versus -15 ms; P = 0.036), with a strong dose-response to reported activity. Adult cartilage can adapt its composition to loading - which is not the same as filling a hole in it.

The certainty of evidence was very low, but there is no measured sign that a run damages the joint surface.

## How much does MACI surgery using your own cartilage cells cost?

There isn't one set price. MACI means matrix-induced autologous chondrocyte implantation. One operation takes cartilage cells from your knee, and a lab grows them. Another operation places those cells into the damaged spot. Ask for a written estimate covering both operations, anesthesia, the facility, later visits, and rehab.

## Does insurance cover MACI using your own cartilage cells?

Coverage depends on your plan and the damage shown in your records. Ask whether each operation needs its own approval. For MACI, the product charge pays for the sheet that carries your grown cartilage cells. Ask the surgeon's billing office and your health plan whether that charge is included.
